NOTE: The below forms are to be filled out by committed incoming/transfer or current KU student-athletes ONLY. Incoming student-athletes should fill out the form after their final season of high school competition is complete, if possible. Current KU student-athletes are asked to fill out the shorter "Returning Student-Athlete Questionnaire" before each season of competition.
